Kindly note there is no offline Self Declaration From or template for NEET UG as far we know and we don't want to mislead any candidate with the information, so please check the NEET website for right information.

How to fill the NEET 2022 self-declaration form offline?

Given below are the quick steps to download and fill the NEET 2022 undertaking manually.
  • Go to the official website: neet.nta.nic.in
  • Click on the ‘NEET UG 2022 admit card’ link
  • Log in by filling in the asked details
  • Fill details in the 'online self-declaration undertaking form' and submit.
  • Then click on the 'Download admit card' tab. The NEET 2022 admit card will appear on the screen along with the self-declaration undertaking form
  • Verify the details mentioned on the admit card
  • Download the admit card and take two-three printouts of all the pages
  • Fill the details asked in the NEET 2022 self-declaration undertaking manually with a blue pen
  • Affix a passport-size photograph as uploaded in the NEET 2022 application form.
  • Get the NEET 2022 admit card undertaking signed by the parents/guardian
  • A left thumb impression and signature on the NTA NEET 2022 undertaking will also be put upon reaching the exam centre in front of the invigilator.

The candidate will be required to bring the following to the Examination Centre: 


 Admit Card along with Self Declaration (Undertaking) 
 Additional passport size photograph same as uploaded on Application Form, which you need to paste on Attendance Sheet 
 One Postcard size (4’’X 6”) color photograph affixed on the designated space (page 2) downloaded from the NTA website (a clear printout on A4 size paper) with white background should be pasted on the Proforma downloaded with the Admit Card and should be handed over to Invigilator at the Centre duly filled in 
 Any Valid Photo Identification Proof issued by the government” – PAN card/ Driving License/ Voter ID/ 12th Class Board Admits or Registration card/ Passport/ Aadhaar Card (With photograph)/ EAadhaar/Ration Card/ Aadhaar Enrolment No. with Photo. All other ID/Photocopies of IDs even if attested/scanned photo of IDs in mobile phone will NOT be considered as valid ID Proof), 
 Personal transparent water bottle. 
 Personal hand sanitizer (50ml). 
 Before reaching the Centre, the candidates must enter the required details in the Undertaking in legible handwriting. 
 You need to carry PwD Certificate, if applicable Note: A candidate who does not bring the downloaded proforma with a postcard size (4”X6”) photograph pasted and one passport size photograph will not be allowed to sit in the examination and shall lead to his/her disqualification.
